Never events, as defined by the National Quality Forum (NQF), a national nonprofit coalition of physicians, hospitals, businesses and policy makers, include 28 preventable incidents that should never occur in a hospital. According to the NQF, ‘never events’ are errors in medical care that are clearly identifiable, preventable, and serious in their consequences for patients. Examples include surgery performed on the wrong patient, an infant discharged to the wrong person, or patient death because of equipment failure. With today’s healthcare organization facing increased consumer and regulatory pressure to improve clinical quality combined with declining Centers for Medicare & Medicaid Services (CMS) reimbursements when these events occur, reducing these events should be the highest priority for every healthcare executive.
